What to Bring to your Pet’s First Appointment
If you are scheduling with us for the first time and your pet has been seen at another hospital, please let us know. We will contact your prior veterinarian or veterinary hospital and request your pet’s prior medical records and vaccination history be forwarded to us, prior to your first visit. If this is your first visit to our hospital, you can aid the veterinarian by providing as much information about your pet as you can. Please bring things such as the name of the food you feed your pet, the names of all medications or supplements your pet is being given.
Payment is due and payable at the time of service. For your convenience, we do accept cash, Visa, American Express, Master Card, Discover and CareCredit.
Pet insurance requires the veterinarian be paid at the time of service. After a claim is filed with the insurance company you are reimbursed according to your pets’ policy. We will happily assist you with filing of insurance claims for your pet’s veterinary care. Please bring your pets insurance claim forms and any other information regarding your pet’s insurance coverage.
